b3ddc5e76f LockedPool: avoid quadratic-time allocation
Use separate maps for used/free chunks to avoid linear scan through alloced
chunks for each alloc.

4536148b15 support: Add LockedPool
Add a pool for locked memory chunks, replacing LockedPageManager.

This is something I've been wanting to do for a long time. The current
approach of locking objects where they happen to be on the stack or heap
in-place causes a lot of mlock/munlock system call overhead, slowing
down any handling of keys.

Also locked memory is a limited resource on many operating systems (and
using a lot of it bogs down the system), so the previous approach of
locking every page that may contain any key information (but also other
information) is wasteful.

d7d187e8a4 allocators: split allocators and pagelocker
Pagelocker is only needed for secure (usually wallet) operations, so don't make
the zero-after-free allocator depend on it.

92fd887fd4 tests: add a BasicTestingSetup and apply to all tests
Make sure that chainparams and logging is properly initialized. Doing
this for every test may be overkill, but this initialization is so
simple that that does not matter.

This should fix the travis issues.

ad49c256c3 Split up util.cpp/h
Split up util.cpp/h into:

- string utilities (hex, base32, base64): no internal dependencies, no dependency on boost (apart from foreach)
- money utilities (parsesmoney, formatmoney)
- time utilities (gettime*, sleep, format date):
- and the rest (logging, argument parsing, config file parsing)

The latter is basically the environment and OS handling,
and is stripped of all utility functions, so we may want to
rename it to something else than util.cpp/h for clarity (Matt suggested
osinterface).

Breaks dependency of sha256.cpp on all the things pulled in by util.

3fc6846181 Add licenses for tests and test data
- Add license headers to source files (years based on commit dates)
  in `src/test` as well as `qa`
- Add `README.md` to `src/test/data` specifying MIT license

51ed9ec971 Cleanup code using forward declarations.
Use misc methods of avoiding unnecesary header includes.
Replace int typedefs with int##_t from stdint.h.
Replace PRI64[xdu] with PRI[xdu]64 from inttypes.h.
Normalize QT_VERSION ifs where possible.
Resolve some indirect dependencies as direct ones.
Remove extern declarations from .cpp files.

e95568b78d Handle locked pages more robustly (Fixes issue #1462)
Memory locks do not stack, that is, pages which have been locked several times by calls to mlock()
will be unlocked by a single call to munlock(). This can result in keying material ending up in swap when
those functions are used naively. In this commit a class "LockedPageManager" is added
that simulates stacking memory locks by keeping a counter per page.
